Please read the instructions carefully before you submit the application form to avoid inconvenience.

For speedy acceptance of application please ensure that the same is submitting duly filled in all respect kindly go through the check list.

Have you written your name in the application form as it appears in your SSC / Hr. Sec. / Diploma / Degree Certificate. Please write in English and Hindi too.

Have you entered your Father's Name (in relevant column).

Have you compared your name, Father's Name / Spelling of the name given in various certificate including date of birth certificate.

Note :

If there is any discrepancy in the spelling or body of name, you have to get that incorporated in all the relevant Certificate of yours from concerned / competent authorities in authenticated manner.

Have you written address in capital letter, with completed pin code? Please mention your latest mailing address, and ensure that the same address is available in the records or State Council / Boards.

Have you entered your date of birth in Christian era? Have enclosed Original Date of Birth Certificate and Two Attested Copies, as proof of age?

Have you enclosed the Original Degree / Diploma ad Matriculation Certificate along with two attested copies?

Have you entered the name of the authority (University / Board / Council) in the same manner as it appears in the Degree Certificate.

Have you enclosed passport size copy of your recent photograph (duly attested) as required?

Have you enclosed the Bank Draft / I.P.O. in the name of the Central Council Of Homoeopathy, New Delhi for an amount of Rs.1000/- (payable at Delhi). Please write the applicant's name on the reverse side of Bank Draft / or in space in I.P.O..

Have you written the Bank Draft / I.P.O. number in the column of the application form?

Have you mentioned registration number / reference number of the State Council / or Board in prescribed column of the application form and attested copies (two)?

Please ensure that all documents have been attached with the application form duly pin up or tagged.

In case you are personally coming to submit your application please bring all the certificates for verification.

Please send the original Degree Certificate and Date of Birth Certificate by registered post or bring it in person. As an alternative you can send it through any person authorized (along with authorization letter) on your behalf; this person can collect the said certificates after verification by the Council.

The person attesting the copies of certificates should identify himself clearly by his name & designation (self attestation can not be entertained) Oath form has to be attested by a registered Homoeopath (and seal of attesting practitioner must indicate his / her qualification, registration number and address.)

To avoid inordinate delay the Council shall return all incomplete applications.

Kindly ensure that the copies are clear and the name, date of birth, name of the qualification, signature of the issuing authority are clearly apparent. No overwriting on the Xerox copy is permissible.

The application must ensure that the name indicated on the application form should be the same as given on the degree as well as the date of birth certificate. In case of any discrepancy form is likely to be rejected.
